For everyone
1.
First tell the police. At a MeeSeva centre, apply for the police "Missing or Lost Documents or Articles" service and keep the receipt. A copy of this police report is needed for a new voter ID card.

If you do not remember your Aadhaar number, find it with the "Retrieve EID/UID" service on the myAadhaar portal; an OTP comes to your linked mobile. You can also call 1947.
Download e-Aadhaar on the myAadhaar portal. It is as valid as the Aadhaar card. The PDF password is the first 4 letters of your name in capitals, followed by your year of birth.
If you want a plastic (PVC) Aadhaar card, order it on the myAadhaar portal with "Order Aadhaar PVC Card" and pay online. It comes by Speed Post to the address in your Aadhaar.
⏱️ within 10 days · Normally within 10 working days (not counting the day you order).

Which document did you lose? Aadhaar card · Is your mobile number linked to your Aadhaar? No, or I am not sure

If your mobile is not linked, go to an Aadhaar Seva Kendra, or to an Aadhaar enrolment centre in a bank, post office or government office. Give your Aadhaar number and get your Aadhaar downloaded and printed. If you do not remember the number, call 1947 first.

8.
Once the Electoral Registration Officer (ERO) approves it, a new card is issued. If you later find the old card, return it to the ERO.

9.
Apply for a reprint of the PAN card on the website of the agency that handled your latest PAN application: onlineservices.proteantech.in for Protean (formerly NSDL), or pan.utiitsl.com for UTIITSL. Your PAN and details stay the same. The card is sent to your address as the Income Tax Department has it.

💰 Fee: ₹50 · For an address in India, taxes included.
Not confirmed: These two agencies' websites are not gov.in sites; they are the agencies that handle PAN services for the Income Tax Department. If you do not know which one handled yours and one site does not accept it, try the other.
10.
If you got your PAN through "Instant e-PAN" with Aadhaar, you can download the e-PAN on the Income Tax e-Filing portal: "Instant E-PAN", then "Check Status / Download e-PAN", with your Aadhaar number and an OTP.
At a MeeSeva centre, ask for a print of your ration card with the "Print ration card" service.

If the PVC Aadhaar card has not come within 10 working days, or for any other Aadhaar problem, call UIDAI's toll-free number 1947, or e-mail help@uidai.gov.in.
If the reprint site does not accept your PAN (because another agency handled your latest application), or no OTP comes because your PAN record has no mobile or e-mail, you must apply with the "PAN Change Request" form.
For the police lost-documents report, MeeSeva charges ₹80, including GST; for the other MeeSeva services here, ask what it costs before you pay. The centre must give you a printed receipt for every payment and display the chart of charges. Charging more than the set amount is treated as fraud: the centre can be fined, and removed if it keeps happening. If you are asked for more, ask for the receipt and complain to the Parishkaram call centre (1100, or toll-free 1800 425 1110). Do not pay agents who offer to speed it up.
