Getting lost documents again

What to do, from the police report to a new copy, when you lose your Aadhaar, voter ID, PAN card, rice card, a certificate, driving licence, vehicle RC or passport.

At the village or ward secretariatOnline

For whom: Anyone in Andhra Pradesh who has lost an important document. If you lost more than one, go through this guide again for each.

Tell us your situation first; then the documents and steps open.

    3Your steps

    · Do them in order. Tick each one when it is done.

    1. Do this now

      First tell the police.

At your village or ward secretariat (now the Swarna Gramam / Swarna Wardu office), apply for the Home Department service "Missing / Lost Documents / Articles" and keep the receipt. Or log in to the AP Police portal with your mobile number and OTP, send a complaint under "Submit A Complaint", and note the Petition ID. A copy of this police report is needed for a new voter card and for a duplicate RC.

Your village or ward secretariat (Swarna Gramam / Swarna Wardu office)

Not confirmed: We could not confirm what the secretariat charges for this service or how many days it takes (we did not open the AP Seva portal, whose certificate is revoked). We could not see whether the online complaint offers a "lost documents" subject; that list comes from a server that refused our requests. Ask at your police station whether you can also report it there, and get a receipt for anything you pay.

    2. Do this now

If you do not remember your Aadhaar number, find it with the "Retrieve EID/UID" service on the myAadhaar portal: give your full name as in Aadhaar and your linked mobile, enter the OTP, and the number comes by SMS.

It is free. You can also call 1947.

Download e-Aadhaar on the myAadhaar portal.

It is as valid as the Aadhaar card. The PDF password is the first 4 letters of your name in capitals, followed by your year of birth.

    4. Do this now

If your mobile is not linked, go to an Aadhaar Seva Kendra, or to an Aadhaar enrolment centre in a bank, post office or government office.

Give your Aadhaar number (or the EID on your enrolment slip) and confirm with a fingerprint or iris scan, and they print your Aadhaar. If you do not remember the number, they can search with your name, gender and district or PIN code. Secretariats also offer an "e-Aadhaar download and color print" service.

Fill in Form 8 for a new voter ID card.

Tick the box for a replacement EPIC without corrections. Give your name, EPIC number and mobile number, and attach a copy of the police report or FIR.

Not confirmed: The Form 8 guidelines are on the Election Commission's portal, but we did not see the online application screens. If you cannot apply online, ask your Booth Level Officer (BLO) or the Electoral Registration Officer (ERO) office of your constituency.

    8. Do this now

Once the Electoral Registration Officer (ERO) approves it, a new card is issued.

If you later find the old card, return it to the ERO.

    9. Do this now

Apply for a reprint of the PAN card on the website of the agency that handled your latest PAN application: onlineservices.proteantech.in for Protean (formerly NSDL), or pan.utiitsl.com for UTIITSL.

If you got your PAN through "Instant e-PAN" with Aadhaar, either one will do. An OTP comes to the mobile or e-mail in your PAN record. Your PAN and details stay the same. The card is sent to your address as the Income Tax Department has it.

      Where
      You do this yourself
      Fee
₹50 · For an address in India, taxes included (₹959 for an address abroad).

Not confirmed: These two agencies' websites are not gov.in sites; they are the agencies that handle PAN services for the Income Tax Department. If you do not know which one handled yours and one site does not accept it, try the other.

    10. Do this now

If you got your PAN through "Instant e-PAN" with Aadhaar, you can download the e-PAN on the Income Tax e-Filing portal: "Instant E-PAN", then "Check Status / Download e-PAN", with your Aadhaar number and an OTP.

    11. Do this now

Losing the card does not stop your ration.

At the fair price shop, give the rice card number or your Aadhaar number and confirm with your fingerprint, iris or face to draw your ration. If the biometric check keeps failing, the Deputy Tahsildar (Civil Supplies), the Mandal Civil Supplies Officer, the VRO or the Ward Revenue Secretary confirms it instead.

    12. Do this now

For a new card, the Collector (Civil Supplies) or an officer they authorise makes an enquiry and issues a new card on the fee the government sets.

There is no fee if the card was lost in a fire, flood or other natural calamity. If you find the old card later, return it to that officer at once.

      Where
Your village or ward secretariat, or the Tahsildar office (Deputy Tahsildar, Civil Supplies)
      Fee
The fee the government sets; free if lost in a natural calamity.

Not confirmed: This rule is from a 2018 order (amended up to 2022). The secretariats' and the Civil Supplies Department's lists of services have no "duplicate rice card" service, and we found no fee. Ask at the secretariat or the Tahsildar office where and how to apply.

    13. Do this now

If you lost a certificate issued through the secretariat or MeeSeva, ask at your secretariat for a fresh print with the "Print Meeseva Certificate" or "Reprint of Certificates" service.

Take the old certificate or application number if you have it.

      Where
Your village or ward secretariat (Swarna Gramam / Swarna Wardu office)
      Fee
A service charge may apply; we could not confirm the amount.

Not confirmed: The names of these services are from the list of secretariat services on a district website. We found no official word on which certificates they cover, the charge, the time or the details needed (we did not open the AP Seva portal, whose certificate is revoked). Taking the certificate number is our own pointer. Ask at the secretariat and get a receipt.

    14. Do this now

On the Sarathi portal for Andhra Pradesh, apply for a duplicate driving licence under "Services on DL (Renewal/Duplicate/AEDL/IDP/Others)" (Form 2).

Attach a copy of the old licence if you have one. Pay the set fee and user charges online.

      Where
      Online
      Fee
The fee and user charges the portal shows.
      Open the official page

Not confirmed: The duplicate licence fee is not on the pages we read; pay what the portal shows. We found no official word on whether Andhra Pradesh asks for a police report (Parivahan's list does not); keep one ready. The Transport Department's own page links to its older portal, which we could not read.

      Sources: 14, 15

    15. Do this now

Tell the Transport Department's registering authority in writing that the RC is lost.

For a duplicate RC, give Form 26 with the police report and the pollution under control (PUC) certificate. If the vehicle is on a loan, the financier must sign Form 26. The fee is half the registration fee: ₹150 for a motor cycle, ₹300 for a private car (non-transport light motor vehicle).

      Where
Transport Department office (registering authority, RTA)
      Fee
Half the registration fee: motor cycle ₹150, private car ₹300.

Not confirmed: The Transport Department's pages are undated and older than the Vahan portal; we did not see whether you now apply online (Vahan) or what the user charge is. Parivahan says some states also ask for the insurance certificate, proof of address or an affidavit. Parivahan says the last registering authority; the AP page says the authority where the vehicle is. Ask at your RTA office and get a receipt for what you pay.

    16. Do this now

If your passport is lost, apply on the Passport Seva portal for a re-issue and choose "Lost/Stolen Passport" as the reason.

The portal's Document Advisor shows which documents you need.

      Where
      Online
      Open the official page

Not confirmed: We could not read the documents, the fee or whether a police report is needed for a lost passport: on 1 November 2026 Passport Seva's instruction booklet and fee rules answered "not found" (404). Check on the portal before you apply, or ask the Passport Seva call centre.

    If something goes wrong

    If it is late

If the PVC Aadhaar card has not come within 10 working days, or for any other Aadhaar problem, call UIDAI's toll-free number 1947, or e-mail help@uidai.gov.in.

    1947

    If it is rejected

If the reprint site does not accept your PAN (because another agency handled your latest application), or no OTP comes because your PAN record has no mobile or e-mail, you must apply with the "PAN Change Request" form. The PAN card guide has the agencies' details.

    If it is late

If an application made at the secretariat (the police report, a certificate print, the rice card) does not come, file a grievance on PGRS: at the secretariat, at the Collectorate on Monday, on the PGRS website or app, or by calling 1100. You need your Aadhaar number to file it.

    1100

If you are asked for more money

Ask for a receipt for every payment. An Aadhaar centre may charge only ₹30 for a print, and the PVC card costs ₹75; if you are asked for more, complain on 1947. If anyone asks for a bribe, call the ACB toll-free on 1064 or e-mail complaints-acb@ap.gov.in. Do not pay agents who offer to speed things up.
